Set in episode 2x22 'As I Lay Dying'. What if Elena's death had a consiquence and the witches were mad at her because she wasn't supposed to live or be human after the sacrifice ritual, and the only way to stop it was one way out? Elena living after dying has sent her mind into insanity and she is hallucinating nothing but bad memories and horrendous amounts of blood.
