This is only my little anal idiosyncracy, but the best disclaimers are those that are formal, precise, and to the point. You don't need to justify yourself to the reader before you begin. They began reading because you caught their interest. :)

As for the story itself: This is a brilliant and chillworthy idea. So good that I actually think it's wasted in a story this short. As far as technical ability goes, you've obviously got the goods so I strongly recommend that you work it over into maybe a two or three-chapter story.

And don't, please don't, conclude stories with an additional disclaimer. It's really not necessary. It's actually quite jarring for the reader. I was still dealing with the shivers running up and down my spine, and the addendum kind of spoiled it for me.
