Enjoying this story idea. Haven't seen the idea before, and I love the idea of Severus having a sister. The vampire idea is even better.

Maybe you could get into her head a bit, explain to the reader why she's behaving as she is, and what is happening to her with these changes.

I don't understand about the sealed documents, though. Knowing how they are about the werewolves I was wondeing why they wouldn't be the same with vampires, even if they aren't changed yet. also wondering why the nurse would be unable to detect it. If she's dealing with a lycanthrope, it seems she wouldn't likely be in the dark of symptoms and scans and etc...

Are you going to follow canon with the deatheaters, as it appears, or have Severus find out how purebloods feel about half-humans and split him from them? Would like to see the Marauders taken down a peg or two.

Looking forward to seeing where you're going to take this.
