It's "she and Karen", not "her and Karen"; "Toby and she", not "Toby and her". When those are the subjects of the sentences ("Toby and she stopped short", "she and Karen had never seen eye to eye"). If they're the objects, then it's "He waited for Karen and her", or whatever. And "as Karen said,", not "like Karen said". Just FYI.
