A/N: I realize Dean makes split second decisions all the time. But he fell through rotten floorboards. And is scratched to hell. And maybe hit is head a little. He just needs a second to think.

"Was it rusty?"

Dean needs to learn to think faster.

A moment of deliberation earns him a visit to the local ER. Sam's worried about bacteria, even though, in retrospect, Dean's pretty sure the nail was clean.

"When was your last tetanus shot?" It takes Dean a second to orient to the intern's question.

The pause is interpreted as uncertainty and he finds himself on the receiving end of a sizable hypodermic.

The injection hurts more than the puncture wounds and Dean's still rubbing his arm when Sam comes in. "You ready to get out of -"

"Yes!" No hesitation.
