Now, the whole world believes that it was Anakin sending the order. They somehow didn't notice the setting of his being in the Jedi Council. Somehow, they didn't notice the inflection of the words, "Execute Order 66", since it was originally said in a sentence, rather than an order by itself.

They are also sure that it isn't Vader who sent the order. What does this mean? It means that there is a large enough difference between Vader and Anakin visually. This, in turn, implies that the command is not keyed to Vader, but what Vader looked like when he was young. Which means Vader didn't take into account that he'd look older as time goes on.
