Firstly: I don't know whether I should hug you or try to strangle you for starting yet another new story. :D

But apart from this, you once again did an amazing job at writing. I already like story from the first chapter on. I'll add it to my favourites :)

There are some things which are still unclear for me: how old is Harry? Which year in Hogwarts did he just finish? (One can't really trust Kakashi's estimation as Harry usually looks younger, since he's so small, than he actually is...) As Sirius is obviously dead it should at least be after fifth year...

Why did he leave the wizarding world? Is there a back story? I mean he decided to flee to Diagon Alley rather than contacting Dumbledore. And his friends?

As for the Hidden Villages... As Kakashi thought Harry to be Sasuke, the time frame has to be at least post-'retrieve Sasuke mission'...

Who is Harry's relative? Are you going with the often used idea of making him related to the Yondaime and Naruto, or do you have another idea? Perhaps make him an Uchiha? A Sarutobi?

How is Harry going to get his wand back if the Sound took it away and he left without it? Will he be able to use Chakra and jutsus?

As far as the pairing is concerned... I REALLY liked the Harry/Itachi in 'Dreams', but since you already brought him together with Kakashi why not just go with this pairing, since there hasn't really been any concrete development with this constellation in SWATTP yet.

But depending on how old you make Harry, the age difference between him and Kakashi may make this a little bit difficult. If he's the same age as Naruto & co, maybe you could pair him with one of the Rookie 9(12). Maybe a Harry/Neji (both lost parents)... I don't think a Naruto/Harry would work. And don't get me started on Harry with one of the Kunoichi of Konoha...
