The disease is most likely tuberculosis, as it's consistent with all the symptoms that she showed. Historically, it was Okita Souji who died of TB while his sister Mitsu looked after him. I guess in Gintama they switched this around so that he wouldn't have to die since he's much more important to the plot than she is. TB is notoriously hard to treat, and while you don't hear about it much today, it used to be a major problem in the past. I don't think there are any other chronic lung conditions that she could have contracted.
