The title of noble house can be bestowed by a vote of ALL the Ancient AND Noble Houses on a minor house for any reason, as long as they have a clear majority vote.
The title of ancient is bestowed when a founded house has had a surviving member for at least five centuries, and at the time of bestowment, has an active head and a fertile and healthy accepted heir (i.e. complies with rules of succession set by the founder of the house)
The title of Most Ancient is bestowed when a founded house had a surviving member for at least a millenium, and insert other requirements for ancient.
The title of Most Noble can only be granted by the Elder Houses through a majority vote.
A house is considered 'founded' when a head establishes a magical 'law' of sorts over his/her house, and formally declares his/her house a part of the Ministry, AND is accepted by at least one Most Ancient and Most Noble House.

Hereditary Wizengamot acts as the Upper House, presided over by the Chief Warlock, and the Elected Wizengamot acts as the Lower House, presided over by a Speaker of the House.
